If I switch on a rule today, do all my regulars get a card at once?

No. A new rule counts from today. Somebody who has already ordered on two hundred days does not wake up to a pile of cards, they get the next one they earn. That is deliberate, because finding out otherwise is an expensive way to learn. The page tells you the same thing when you save the rule.
