Why was my QR code colour rejected when I saved it?

Contrast. The colours you picked did not leave enough difference between the code and its background for a phone camera to read reliably, so the save is refused rather than allowed through. A code nobody can scan costs a whole print run to discover, which is why this is a refusal and not a warning.
